What to Expect
The Singles Database questionnaire has around 35 questions. One thing worth knowing before you start it: you won’t be able to pause halfway, save it, and finish later — it needs to be completed in one sitting.
That’s exactly why this page exists. We want to give you an opportunity to think about your answers ahead of time, so the form itself becomes quick: you’re just typing in what you already know.
Before you begin, have these ready
A current photo of yourself. Clear and recent. You’ll upload it during the questionnaire.

An email address for the invoice payment. This can be your email, or the email address of a family member/support person if they agreed to pay the $350 fee.
